Insurance and Billing FAQ
We are extremely pleased that you have chosen us for your dermatological needs. The following information regarding financial matters should be helpful to you in understanding our billing process.
INSURANCE PLANS: Dermatology Specialists of Omaha, LLC participates with all major local and national health plans. However, an occasional health plan may not offer participation to physicians in the area. It is important you check to make sure the provider you are seeing is a participating provider with your health plan.
DUE AT TIME OF CHECK-IN: All co-payments and non-covered charges (those considered to be cosmetic by your insurance company) are due at the time of check-in.
MEDICARE: Some Medicare services may not be considered medically necessary by Medicare. We will inform you before the procedure in those situations.
COSMETIC SERVICES: Appointments for cosmetic services will REQUIRE a $50 deposit at the time the appointment is made. All cosmetic services and/or purchases are paid at the time of service will not be filed with your insurance company.
REFERRALS: If you belong to an HMO or POS, you are responsible for getting a referral from your primary care doctor. If you do not have the referral at our office at the time of your appointment and we are unable to obtain one for you, you will be asked to sign a waiver stating you will be responsible for the payment of the visit, or, you may reschedule your appointment.
MONTHLY STATEMENTS: You will receive a monthly statement each month once your insurance company makes a payment on your services. It will show the amount charged and the payments made. You will continue to receive a statement or a telephone call until the amount due by you is paid.
PATHOLOGY SERVICES: If you have a biopsy taken, you or your insurance company will be billed a fee for processing and interpreting the slide. Drs. Marc Hapke and Dominick DeMaio are members of our group. They are board certified anatomical pathologists and dermatopathologists. In some instances a second opinion or additional stains may be required to make a final diagnosis for your slide. You may be billed by either Regional Pathology at the Nebraska Medical Center or Creighton Medical Labs. Your insurance company may assess an additional co-payment for any lab or pathology services.
LABORATORY SERVICES: If you have blood drawn , you or your insurance company will receive a bill from either Regional Pathology at the NMC or Creighton Medical Labs. If your insurance company requires a specific laboratory for processing of your blood work, it is your responsibility to notify the clinical staff at the time of the blood draw.
FINANCIAL ASSISTANCE: We offer financial assistance for our patients who don’t have insurance coverage. If you need assistance, please feel free to discuss it with our billing supervisor by calling (402) 933-4123.
PAYMENT ARRANGEMENTS: If you are having difficulty making payments on your account, please feel free to discuss it with us. In many situations, we will set up a payment plan that will fit your budget. Please feel free to contact our billing supervisor at (402) 933-4123.
NO SHOW/LATE CANCELLATION FEE: If you are unable to keep your scheduled appointment we REQUIRE a 24 hour notice . If you do not a $25 No show/late cancellation fee may be billed to you.
INSUFFICIENT FUNDS: If you have paid your bill with a check and it is returned to us as insufficient funds, we will charge your account a $30 service charge. In addition, DSO contracts with checXchange. Should you issue a check to us it authorizes your account to be electronically debited for the amount of the check plus any applicable fees. The use of a check is your acknowledgement and acceptance of this policy and its terms. You will be asked to pay in cash for your on-going visits.
